At Elite Oral & Facial Surgery we strive to provide the best possible experience and we don’t want finances to be the reason patients are unable to receive the care they deserve. Our benefits and treatment coordinators are knowledgeable and can help maximize your dental benefits to minimize your out-of-pocket cost. We review all of the choices available to help pay for treatment. All of our doctors are participants in most major PPO dental plans! Insurance & Discount Plans

To save you time and hassle, we accept many dental insurance plans and will file claims on your behalf. Our knowledgeable benefit coordinators can help you maximize your dental benefits and minimize your out-of-pocket cost. We inform you prior to any appointments or procedures exactly what your insurance plan will pay for and offer options for the payment of any remaining balance.

Insurance FAQ

Treatment that is recommended by a dentist, is listed on the fee schedule, and accepted under the terms of your group’s plan.

Treatment that is either not listed on your fee schedule or more than the minimum to restore the tooth back to its original function.

Indemnity or Traditional Insurance reimburses members or dentists at the dentist’s UCR (Usual, Customary & Reasonable fee). This allows the subscriber to go to any dental office without being limited to a panel.

PPO

(Preferred Provider Organization) is the most common form of insurance. They provide members with a list of participating dentists to choose from. The dentists on this list have agreed to a lower fee schedule, which provides you with greater cost savings. They also assist with insurance billing. Most companies pay 50% to 80% on major treatment (simple & complex extractions, frenectomy) and up to 80% for preventative care (consultation exams, x-rays, CT scan). Annual maximums generally range from $1,000 to $2,000.

HMO

Also known as capitated or prepaid insurance, was designed to provide members with basic care at the lowest rate. Participating providers receive a monthly capitation check for patients assigned to the office. This amount is only a few dollars and is intended to offset the administrative costs. HMOs generally don’t pay for services rendered. Fees are usually greatly reduced, but the patient is solely responsible for paying the doctor.

Discount Plan

Dental savings plans – sometimes called “dental discount plans” – are an affordable, flexible alternative to traditional dental insurance. Think of them as a buyer’s club for your teeth, you join the club and get member-only discounts. With a dental saving plan, you can expect savings of 10-60% on specialty dental care. We believe that everyone deserves to have access to high-quality dental care and procedures. That’s why for more than 10 years, we’ve been committed to making that care more accessible and more affordable for everyone.
